Focus Point Sponsored Residential provides excellent services and supports to youth and to individuals with developmental and intellectual disabilities throughout the Commonwealth of Virginia. Focus Point Sponsored Residential believes that individuals we support should have access to professional resources that allow them to take charge of their lives and allow them to make decisions about their futures. Through its continuum of supports, Focus Point Sponsored Residential follows this principle. As part of our Sponsored Residential Programs, we provide a comfortable and warm environment where individuals are able to make choices in daily living activities, and are professionally supported to plan their future with the support of their families and friends. An individual's strengths, capabilities, preferences, lifestyle, and cultural background are considered during person-centered planning. With these services and supports, the individual is provided with an environment that allows them to maximize their quality of life and growth. A Focus Point Sponsored Residential Sponsored Home focuses on the individual and addresses their needs for choice, relationships, lifestyle, health, well-being, rights, and satisfaction.

Providing 24-hour residential and individual support for one or two adults with intellectual disabilities, Focus Point Sponsored Residential Sponsored Homes offer 24-hour residential and individual support. It is possible that these individuals are transitioning from a natural family environment or from a program that has just closed or been downsized. We have developed an admission policy that identifies the criteria for admission considerations and provides assistance to Sponsored Homes.

In Focus Point Sponsored Residential Sponsored Homes, each person supported teaches self-advocacy, choice, healthy life habits, behavioral self-management, and also develops critical life skills necessary to function independently in the immediate and community environment. Every individual has an Individual Service Plan (ISP) developed through a person-centered process. In the Individual Support Plan, progressive steps are outlined that are based on what is important to each person supported as well as what they want to achieve.

In Focus Point Sponsored Residential Programs, parents/family/guardians play an integral role in overall progress and adjustment. It is important to maintain continuous communication between the family and the home in order to keep them informed and to ensure that the services and supports developed for them are maintained. It is imperative to provide services that are person-centered, especially when people are transitioning into new environments.

Sponsored Residential Program at Focus Points The Sponsored Residential program promotes a person-centered philosophy.  Our responsibility is to help each individual served live a productive and fulfilling life by utilizing resources we have at our disposal.  For individuals to develop their talents, the facility believes that they must be welcomed, celebrated, listened to, challenged, and supported in every environment. It is imperative that individuals are encouraged and supported to make their contributions by being aware of themselves and encouraging them.
Our Sponsored Residential program provides a wide range of services to a variety of communities throughout Virginia. Families/individuals who are interested in becoming Sponsored Families are also selected with pride to provide these supports to individuals with developmental and intellectual disabilities. In order to qualify for these services, a number of requirements and steps must be met. Focus Point Sponsored Residential is here to assist interested and qualified families throughout the process.
 
Among the first steps is the completion of a Home Study. Focus Point Sponsored Residential Associate Directors and/or Sponsored Home Case Managers assess potential sponsored homes during the process. As a result of this assessment, Focus Point Sponsored Residential is able to determine if the individual, family, and environment are suitable for supporting an individual with a developmental or intellectual disability, and if licensure expectations and requirements can be met. Focus Point Sponsored Residential requires documentation from potential Sponsored Homes to ensure compliance.
 
Our Sponsored Residential Homes will provide services through Focus Point Sponsored Residential. Below is a list of some of the services offered to sponsored families, but is not intended to be comprehensive or complete:
During the pre-placement and application process, assist the potential Sponsor by remaining available and checking in throughout;
 
  • Identify, access, and complete the orientation training and ongoing annual training for the new Sponsor;
  • In addition to helping you prepare for and move the person into your home, we will be present to assist you;
  • As a minimum, we will communicate with you/see you weekly for the first 60 days after an Individual moves in, and we promise to be available whenever you need us;
  • We will contact you/see you at least monthly after your first 60 days, with the commitment of being there to assist you as needed;
  • A review of the service delivery and the program documentation will take place to ensure you are comfortable with the provision of the services as well as to identify any additional support you may need.
  • Our commitment to be available to you also means being available to you to ensure the people we share responsibility for live in an environment that promotes dignity, respect, support, and dreams.
