Documentation Requirements for New Referrals:
To ensure a smooth intake process, Focus Point Homes requires the following documentation from the assigned case manager or support coordinator:

Admissions Process
At Focus Point Homes, we strive to make the admissions process smooth, supportive, and tailored to the unique needs of each individual. Here’s how it works:

At Focus Point Homes, our admissions process is designed to prioritize comfort, choice, and collaboration every step of the way. Let us help you or your loved one find the support and care they need to thrive.
Initial Meeting
Our admissions staff will schedule an appointment to meet with the individual being referred to learn about their needs, preferences, and goals for placement.
Provider Matching
Based on the information gathered during the initial meeting, our team will identify potential providers whose homes and services align with the preferences and requirements of the individual.
Home Tours
Tours are arranged for the individual and their family to visit potential provider homes. This allows everyone to meet the provider, learn more about the environment, and gain a clear understanding of the services offered.
Trial Visit
To ensure the best fit, a trial visit is scheduled. This gives the individual an opportunity to stay with the provider for a few days and experience the service firsthand.
Service Plan Development
After a successful trial visit, we begin developing a personalized service plan that addresses the individual’s needs and goals. A start date for the service is then established.
Flexible Timelines
We understand that every situation is unique, and we can expedite the admissions process to accommodate the individual’s timeline for placement.

Our qualified program management staff conduct regular service reviews, including unannounced visits at least twice a year and announced visits monthly. Collaboration with families, case managers, and other service providers ensures every individual’s success.

Maintaining close family connections is a priority at Focus Point Homes. Visits are always encouraged, and for those at a distance, we facilitate relationships through pictures, letters, phone calls, and virtual tools like video conferencing.

Our Quality Assurance Team and Regional Coordinators provide 24/7 emergency support to address critical incidents. Providers are guided to first contact local emergency services in medical emergencies, while our team is always available to assist in problem-solving and ensuring continuity of care.
